Creating a Flight Request
A completed and signed Flight Request form is required for every passenger flight flown on state aircraft. This form allows Flight Operations to properly plan and schedule the requested flight, and authorizes the passengers to travel on state-owned aircraft. The signature block signifies that the agency head (or their authorized representative) verifies that the listed passengers meet all state requirements to travel in official state vehicles. When a flight request is sent by e-mail, the e-mail account suffices as the agency head’s authorization and the signature block can be left blank.
The following are general guidelines to assist in the completion and submission of the Flight Request form. In addition, useful samples are provided in the links section below.
1. Determine the desired travel day(s) and the number of passengers seats needed. As a general rule, the maximum passengers for overnight trips is eight, for either aircraft. For day trips, the maximum passengers for N102WY is nine to 10, and for N101WY the maximum passengers is eight.
2. Call Flight Operations at (307) 777-3951 to determine availability of the aircraft.
3. Select one of the following Flight Request forms to submit:
(a) Excel (xls) form
(b) Adobe (pdf) form
We encourage you to call us at (307) 777-3951 to answer any questions you may have about this form or any other flight needs you may have. Remember, just because both aircraft appear to be busy on any given day as viewed on the Aircraft Flight Schedule, does not mean we are not able to rearrange flight times to meet all our passengers’ needs and to reduce their costs of flying. As a matter of fact, 60 percent of our flights include passengers from multiple agencies, so please call.
