Federal DOT Medical Waiver Certification

You are required to present a valid Federal DOT Medical Waiver Certificate when applying for a CDL license!

  • If you cannot obtain a certificate for whatever reason, you must be restricted to "intrastate driving only" and the "K" restriction code will be added to your license. 
  • If you apply for a handicap placard, you will be required to present a current certificate which addresses the medical condition you have that qualifies you for the placard.
  • If your Federal DOT Medical Waiver Certificate indicates "state waiver", then you must be restricted to "intrastate driving only" and your CDL license must have the "K" restriction code on it.
