Creating a Flight Request

A completed and signed Flight Request form is required for every passenger flight that is flown on the state's aircraft. This form allows the Flight Operations Program to properly plan and schedule the requested flight.  It also serves as authorization for the passengers to travel on the aircraft.  By signing the signature block an agency head, or their authorized representative, is verifying the listed passengers meet all the state requirements to travel on and in official state vehicles, including the aircraft.  When a flight request is sent via email from an agency head, or their authorized representative, the email account serves as the agency head’s authorization, and the signature block may be left blank.                                                

The following are general guidelines to assist in the completion and the submission of the Flight Request form.  In addition, useful samples are provided in the links section below.

1.  Determine the desired travel day(s)

2.  Determine the number of passenger seats needed.

  • As a general rule for day trips, the maximum number of passengers on aircraft 102WY is limited to nine or 10 based on the passengers' weight, and the maximum number of passengers on aircraft 101WY is limited to eight  based on the number of available seats.
  • The maximum number of passengers for an overnight trip is eight  on either aircraft based on passenger and baggage weight.

3.  Call Flight Operations at (307) 777-3951 to determine aircraft availability.

4.  Select one of the following Flight Request forms to submit:

 (a)  Excel (xls) Form

  • If you have Microsoft Excel, download this form to your computer, and fill in the applicable blocks.
  • This form has drop-down menus for each city, or you can type in the city if it does not appear on the list.
  • Detailed instructions for each form block are provided on the Flight Request Form Instructions PDF below.
  • Contact Flight Scheduling or fax the completed form to (307) 637-7352.

(b)  Adobe (pdf) Form

  • Download this form to your computer, and fill in the applicable blocks.
  • This is a fillable PDF Form, which can be completed using Acrobat Reader.
